If you are volunteering to help with an event or project for The Sparrow’s Nest, you will be asked to fill out a Release and Waiver of Liability.
To submit the form, select the appropriate version (for adults or for minors) and do one of the following:
- Turn in a paper copy: Download, print it, fill it out by hand, and mail it to the address shown at the end of the form or bring to the event/project location, if directed.
- Submit electronically: scan the completed application, and email scanned file(s) to info@thesparrowsneststl.org.
